Transition Planning
As discharge approaches, our team collaborates with the patient and their support system, when appropriate, to develop an individualized aftercare plan, which may include:
- Partial Hospitalization (PHP) or Intensive Outpatient (IOP)
- Ongoing individual or group therapy
- Psychiatric follow-up for medication management
- Referrals to community-based resources
